Patient Forms
An average first visit for our patients lasts between 1 to 1.5 hours because we want to ensure we fully understand your oral healthcare history as well as current and future needs. A large portion of the initial visit will be spent reviewing your health history, establishing your current dental health, and engaging in a one-on-one consultation with Dr. Hejazi to plan for your future dental care. We conveniently provide all of our new patient forms online so you can complete them prior to your visit.
